The following Terms and Conditions Agreement {“Agreement”} constitutes a binding contract between you (“the
patient”) and Glimmer Dental Pty Ltd (Glimmer). By submitting a case to Glimmer the patient agrees to by the
following Terms and Conditions. The patient & Glimmer may each be referred to herein as a -Party- or collectively as
the “Parties”.
By using our products and services, you agree to indemnify and hold harmless Glimmer and its affiliates, directors, officers, employees, agents, and contractors from and against any and all claims, liabilities, damages, losses, costs, or expenses (including but not limited to legal fees) arising out of or related to your use of our products or services, including any breach of these terms and conditions, violation of any law or the rights of a third party, or any other act or omission by you. This indemnification shall include, but not be limited to, any damage to property, personal injury, or other harm suffered by any party as a result of your actions or omissions. You agree to cooperate fully in the defense of any such claim and to reimburse Glimmer for all expenses incurred in connection with the defense of such claim. This indemnification provision shall survive termination of your use of our services and products. By using our products and services, you confirm that your dental and oral health and hygiene is in the best shape and you don’t have any loose, damaged, missing teeth, or any oral or dental related issues.
The aligner product is a clear, removable and medical graded plastic appliances (aligners customized to fit the patients’ teeth. When single or both arches are being treated. a pair of aligners (one for each arch} is referred to as an aligner. In single-arch treatment, one aligner constitutes a stage by itself. Impression kit is a set of trays and impression material (called putty) that can be used to take impressions at home. Aligners are manufactured and shipped from our contract manufacturers.
There are two types of aligners, active and passive. The active aligners are designed to apply pressure to specific teeth to induce movement. Retainers, (also known as “passive aligners known mostly as retainers’) are designed to maintain the existing position of teeth.
By submitting an order, the patient authorizes Glimmer to charge a fee associated to the case procedures. If the patient, after ordering the services and products, cancels the order, Glimmer has the authority to deduct 50% of the total payment received from him/her in order to cover the cost expenditures after the case submission. If the patient discontinues the treatment once the treatment has been approved and aligner manufacturing has started, Glimmer has the authority to deduct 100% of the payment in order to cover the cost expenditures.
Patient agrees to comply with the treatment plan provided by the Glimmer, including wearing the aligners as instructed and attending all follow-up virtual appointments (if any).
Wearing Aligners: Patient agrees to wear the aligners for the prescribed number of hours per day (22 hours a day), as directed by the Glimmer, and to switch to new aligners according to the recommended schedule. If nothing is specified, the default wear schedule is to wear a new pair every 15 days.
Oral Hygiene: Patient agrees to maintain proper oral hygiene throughout the treatment process, including brushing and flossing regularly and cleaning the aligners.
Lifestyle Changes: Patient acknowledges that certain lifestyle changes may be necessary during aligner treatment, including avoiding foods and beverages that may damage the aligners or contribute to tooth staining.
Attendance at Appointments: If required, Patient agrees to attend all scheduled appointments with the Glimmer for progress monitoring, adjustments to treatment, and any other necessary services.
Financial Obligations: Patient agrees to fulfill all financial obligations associated with the aligner treatment and any additional services or products.
Compliance with Instructions: Patient agrees to comply with all instructions provided by the Glimmer regarding the aligner treatment, including any recommendations or guidelines for optimal results.
